Guidelines for completing the Manager/HR Referral Form

  1. This form will be used for all referrals from the Manager or HR to the Occupational Health Dr/ Advisor.  This includes Post-Placement health assessments, absence management issues and other work related issues.
  2. The reason for referral MUST be discussed with the employee.
  3. The referring Manager/HR must complete the Employee Details and the Potential Job Hazards sections.  It is imperative that this form documents all the potential hazards likely to be incurred by the employee, as these can affect an employee’s fitness for work.
  4. A copy of the employee’s job description or list of tasks must always be attached to the HR Referral Form.
  5. When completing the Reasons for Referral section please outline the specific information required.  It is important to be aware that this document will form part of the employees health records and, as such, the employee has the right to see this document if they so wish.
  6. If the referral is relating to an attendance problem, a record of the employee’s attendance record and any Fit notes for the previous 12 months should be attached. Any GP or Consultant reports should also be attached.
  7. If the employee is assessed by an OH Physician, under GMC guidelines, they are entitled to see the Dr’s report 48 hours before it is released to the Manager/HR. Please note the employee can withdraw consent of its release, or ask that incorrect facts are changed. In this case an addendum of their comments will be attached to the original report. The professional opinion of the Dr cannot be altered by the employee.
  8. Please ask the employee to bring all their medication with them, in their original containers, together with any Hospital, Physio and Psychiatric etc appointment cards.
  9. If a follow-up appointment is planned this will be indicated on the report.
